No he's not wearing it wrong, in most African countries women carry their kids on the back. This is picture is from Nigeria which I'm from and traditionally this is the style of carrying children, although modern parents would use the front child carriers.
I've carried two of my kids in many different types of carriers. Some were safer than others just based on the positioning of the child. The reason why carriers have evolved to the front is safety standards have changed as people have studied accidents and deaths involved with carriers. Just because its traditional doesn't mean it is a safe way. It can be based on the age of the child, but with young babies, their bodies are a bit more fragile. children in America were being feed cough syrup with morphine. We don't do that anymore. Smoking was considered good for you. Not anymore.
It's actually really safe. You tie the blanket really tight so there's no possible way the baby could fall out. People in Africa have been carrying their babies like this for ages.
